Zusammenfassung Früherkennung und Prävention chronischer Erkrankungen gewinnen angesichts der zunehmenden demografischen Alterung der Gesellschaft und des damit teilweise in Verbindung stehenden Anstiegs der Gesundheitsausgaben an Relevanz. Vor- und Nachteile entsprechender Strategien werden jedoch kontrovers diskutiert. Vor diesem Hintergrund untersucht die vorliegende Arbeit die Kosteneffektivität der Früherkennung und nichtmedikamentösen Prävention des Typ 2 Diabetes mellitus und seiner Vorstufen im Rahmen der deutschen gesetzlichen Krankenversicherung. Die Modellierung auf Basis einer Monte-Carlo-Mikrosimulation belegt die Vorteilhaftigkeit eines solchen Angebots. Die praktische Umsetzung zur Realisation der erwarteten Kostensenkung und höheren Lebensqualität erfordert weitere Anreize. Abstract Considering demographic ageing of societies and the increase in health expenditures, early detection and prevention of chronic diseases is gaining in importance. However, assets and drawbacks of such strategies are controversial. Considering this, the current publication discusses cost-effectiveness of early detection and prevention by lifestyle-intervention of type-2-diabetes mellitus and prediabetes in the setting of Germany’s statutory health insurance. Results of a Monte-Carlo-Microsimulation-Model show the benefits the proposed program. However, there is a need for further incentives before implementation and realisation of the expected cost-savings and higher quality of life.
