Die Transplantation von postmortal gespendeten Organen ist für Menschen mit irreversiblen Organversagen im Endstadium seit vielen Jahren eine lebensrettende und die Lebensqualität verbessernde Therapie [1] [2]. Nicht zuletzt aufgrund fehlender Leitlinien ist das anästhesiologische Management bei der Organentnahme erschwert. Der Beitrag fasst die existierende Literatur zusammen und überprüft, ob evidenzbasierte Empfehlungen für das anästhesiologische Management abgeleitet werden können. Abstract The transplantation of organs from postmortem organ donors has been a lifesaving and quality-of-life-improving therapy for patients with irreversible organ failure for many years. In Germany, however, there has been an imbalance between the number of organs donated postmortem and the number of patients on the waiting list for years. The anesthesiological management of multiple organ harvesting (MOE) in postmortem organ donors is not an everyday challenge for various reasons: A lack of practical expertise due to the small number of MOE, even at university hospitals (usually < 20 per year), complex pathophysiological changes in the cardiovascular system and other organ functions of the postmortem organ donor and the lack of guidelines complicate anesthesiological management. This paper compiles the existing literature and reviews whether evidence-based recommendations can be derived for anesthesiologic management for MOE. Kernaussagen Das anästhesiologische Management einer Multiorganentnahme ist in Deutschland eine seltene und somit nicht alltägliche Herausforderung – auch für erfahrene Anästhesisten. Derzeit gibt es keine literaturbasierte Evidenz für das optimale anästhesiologische Management. Es gibt Hinweise, dass organprotektive Konzepte für kritisch Kranke auch im Kontext der Multiorganentnahme zur Anwendung kommen sollten. Die Verwendung von gesonderten Checklisten kann die kommunikative Professionalität und somit die Patientensicherheit erhöhen. Die systematische Dokumentation des anästhesiologischen Managements sollte zur Qualitätssicherung und weiterführenden Forschung erfolgen. Künftige Forschung zum optimalen anästhesiologischen Management wäre wünschenswert, um die Organqualität zu erhalten und Organverluste zu vermeiden. Die Entwicklung einer nationalen Leitlinie unter der Schirmherrschaft der führenden Fachgesellschaften (s. Übersicht) wäre wünschenswert, um einen deutschlandweiten anästhesiologischen Standard bei MOE zu etablieren.
