Die robotergestützte Ösophagusresektion gehört zu den Hochrisiko-Operationen und bedarf einer aufwendigen Planung und operativ-anästhesiologischen Abstimmung. Aufgrund der Positionierung der Patienten mit intraoperativer Umlagerung ist der Zugang zu den Atemwegen erschwert, was jegliche Manipulation an einem Doppellumentubus limitiert. Ein videogestützter Doppellumentubus kann bei der Abstimmung im interdisziplinären Team sehr hilfreich sein. Abstract Robot-assisted esophagectomies are still considered high-risk procedures requiring complex surgical and anesthesiological planning and coordination. The operative space during the thoracic operative part is created by one-lung ventilation. Due to special patient positioning and intraoperative repositioning maneuvers, however, access to patient airway or double-lumen tube manipulation, if necessary, is only possible to a certain extent. In this work, we present our experiences establishing a video-guided double-lumen tube for esophageal surgery. From our point of view, the use of video-guided double-lumen tubes is very suitable increasing patient safety and coordination in the operational team during esophagectomy. Kernaussagen Die thorakoabdominelle Ösophagektomie wird zunehmend minimalinvasiv und roboterassistiert – eingebettet in ein interdisziplinäres Therapiekonzept – durchgeführt. Die Nutzung eines Doppellumentubus mit integrierter Kamera (videoDLT) ermöglicht einen kontinuierlichen Blick auf die Hauptkarina zur Lagekontrolle in Echtzeit. Dies ist besonders bei intraoperativer Lageveränderung relevant. Die Verwendung von videoDLT kann die Zahl der lagerungsbedingten Bronchoskopien reduzieren und bietet dem Team aus Anästhesie und Chirurgie Sicherheit. Die Nutzung von videoDLT kann trotz hohen Preises für den einzelnen Tubus u. a. aufgrund der hohen Aufbereitungskosten für die Bronchoskopie in der Gesamtbetrachtung Kosten einsparen. Zu den Limitationen der videoDLT-Technologie gehört die eingeschränkte Sicht bei Sekretverlegung, die sich bisher nur ungenügend beseitigen lässt.
