Nosokomiale Pneumonien zählen nach wie vor zu den häufigsten Infektionen, die im Krankenhaus erworben werden. Neben den beatmungs-assoziierten Pneumonien sind auch nicht-beatmete Pneumonien mit einer längeren Verweildauer, mehr Behandlungskosten und einer erhöhten Mortalität assoziiert. Dieser Beitrag liefert einen Überblick zur Epidemiologie, Pathogenese, Diagnostik und Therapie. Empfehlungen zur Prävention folgen dann in Teil 2 dieses Beitrags. Kernaussagen Die Beatmungs-assoziierte Pneumonie stellt nach wie vor eine der häufigsten nosokomialen Infektionen in deutschen Krankenhäusern dar. Zwei Drittel der nosokomialen Pneumonien sind nicht beatmungsassoziiert (NV-HAP) und werden über die bestehende Surveillance nicht erfasst. Die davon ausgehenden Gefahren scheinen derzeit noch unterschätzt zu werden. Eine NV-HAP ist wie eine VAP mit längerer Liegezeit und erhöhter Mortalität assoziiert. Insbesondere bei schwer kranken Patienten ist ein rascher Erregerwechsel im Oropharynx von Erregern der physiologischen Mundflora zu nosokomial bedeutsamen Erregern im Sinne einer Überwucherungsflora sowohl bei VAP als auch bei NV-HAP zu beobachten. Bei beatmeten Patienten stellt der Endotrachealtubus eine Leitschiene für die Mikroaspiration dar. Wenn möglich, sollten nichtinvasive Atemunterstützungsverfahren bevorzugt werden, ohne eine notwendige Intubation unnötig hinauszuzögern. Als wichtige Faktoren für die Entstehung einer NV-HAP gelten mangelhafte Mundpflege bzw. schlechter Dentalzustand sowie Dysphagie mit Mikroaspiration.
