Vitrectomy management of the severely traumatized eye

Here are a few recommendations regarding management: 1) Do not give up on the eye even if it has lost light perception. 2) Do not factor in your management algorithm the possibility of sympathetic ophthalmia. 3) Perform primary wound closure if the eyewall can be reconstructed. 4) Do not force suture-closure of a scleral wound that is too posterior to allow convenient access. 5) Unless the scleral wound is unsutured, consider primary comprehensive reconstruction. 6) If secondary reconstruction is the selected management option, employ heavy topical corticosteroid therapy after the initial surgery. 7) Perform sec…
