BackgroundNew models are needed for the care of the increasing elderly, chronically ill, and multimorbid population (World Health Organization [WHO] 2020). There is a shortage of health professionals in primary healthcare (Merçay et al. 2021). Rural regions are particularly affected and bottlenecks in provision of care are emerging (Barnes et al. 2018; Cerny et al. 2016).
