IntroductionOlder adults with cognitive impairment are frequently admitted to acute care hospitals [28]. A representative study in Germany reported cognitive impairment in 40% of older inpatients [1]. There is strong evidence that hospitalization is associated with harmful consequences in this population, such as subsequent nursing home admission and mortality [28].
