Einleitung Chronic heart diseases are complex to manage due to structural or functional disorders of the heart. Their prevalence is high and those affected require long-term support. However, the increasing age of general practitioners combined with a shortage of young physicians, especially in rural areas in Germany, is having a growing impact on primary care.
